The expression and significance of Survivin in prostate cancer with neuroendocrine differentiation

摘要 目的分析Survivin在神经内分泌分化前列腺癌中的表达与意义,探讨Survivn在雄激素非依赖性前列腺癌进程中的作用。方法免疫组织SP法检测SYN和CgA的表达,以SYN和(或)CgA表达阳性者确诊为具神经内分泌分化的前列腺癌。同时检测Survivin在本组中的表达,观察Survivin在神经内分泌分化的前列腺癌中的表达。结果 (1)28例前列腺癌标本中,SYN、CgA和Survivin的阳性表达率分别为53.6%、50.0%、89.3%,前两者在BPH标本中均表达阴性,Survivin在BPH中表达率为11.1%。(2)Survivin在SYN和(或)CgA阳性的PCa中表达率为95.2%,Survivin的表达与神经内分泌分化呈正相关。(3)前列腺癌中Survivin的表达与患者的年龄无关,而与分期和分级有关。结论 Survivin的表达与前列腺癌的分期和分级相关,Survivin在神经内分泌分化前列腺癌的发生、发展中发挥重要作用,可能是具神经内分泌分化的前列腺癌耐内分泌治疗的原因之一。 Objective To analyze the expression of survivin in prostate cancer with neuroendocrine differentiation and its significance in development of androgen-independent prostate cancer. Methods The expression of chromogranin A (CgA) and synaptophysin (SYN) were detected by Immunohistochemistry. NED are defined as positive express chromogranin A (CgA) and/or synaptophysin(SYN) in cytoplasm of prostate cancer cell. the expression of survivin was detected by immunohistochemically in prostate cancer with neuroendocrine differentiation. Results (1) The positive rates of SYN, CGA and survivin in 28 cases PCa were 53.6%, 50.0%, 89.3% respectively, while the positive rate of survivin was 11.1% and negative of SYN and CgA in 9 cases BPH. (2) The positive rate of survivin was 95.2% in the prostate cancer with NED, and survivin expression was positiveli correlated with NED in the PCa. (3) The expression of survivin was not associated with age, but was closely related to clinical stage and histological grading of PCa. Conclusion The expression of survivin was closely associated with clinical stage and histological grading of PCa. Survivin might play an important role in carcinogenesis and progression of PCa, and might be a potential reason of endocrine-resisted treatment for patients with androgen-independent prostate cancer
