医源性子宫内膜异位症19例临床分析

Clinical Analysis of 19 Patients with Iatrogenic Endometriosis

目的:探讨医源性子宫内膜异位症最佳诊断及治疗方法,预防盆腔外医源性子宫内膜异位症的发生和降低其复发率。方法:回顾性分析医源性子宫内膜异位症19例患者的临床资料。结果:本组手术治疗18例,术后2例复发,为会阴切口瘢痕子宫内膜异位症,加用孕三烯酮或曲普瑞林治疗3～6个月治愈;单纯药物治疗1例为腹壁子宫内膜异位症停药后再次复发。结论:医源性子宫内膜异位症最好的治疗方法是手术切除,不能彻底切除者加用治疗内异症的药物治疗;严格操作规范,提高手术技巧,可避免医源性内异症的发生。

Objective： To explore the reasonably diagnosis and therapy of iatrogenic endometriosis and to reduce the incidence and recurrence rate. Methods： 19 cases of iatrogenic endometriosis were analyzed retrospectively. Results：Among the 19 cases, 18 were treated by operations, and 2 cases of endometriosis in perineal incision recurred and cured by used gestrinone or triptorelin for 3-6 monthes.The other 1 cases of endometriosis in abdominal wall was only treated with drugs end of dose failure. Conclusion： The best method to iatrogenic endometriosis outside pelvic cavity is surgery. The combination of surgery with drugs can prevent recurrence effectively to the cases which surgery can not completely remove lesions. Strict rules to improve surgical techniques is the best way to prevention of iatrogenic endometriosis.
