围手术期留置尿管会阴擦洗患者真实体验的质性研究

The real -life experiences of peri- operative indwelling catheter in patients with perineal scrub

目的探讨患者留置尿管期间的心理感受，分析相关应激源对患者的影响，为护理工作者制定针对性处理措施，指导临床实践，提供依据。方法采用现象学研究方法探讨10例留置尿管患者的真实体验，以半结构式的深入访谈取得访淡资料，对访谈资料以缩减、分类、诠释、提炼的方法进行分析，提炼主题。结果访谈共提炼出5个主题：缺乏留置尿管会阴擦洗的相关知识；精神紧张，情绪波动、烦躁；自身痛苦不被重视和理解；自卑和无助；操作环境对患者感受有影响。结论操作者应重视患者主观感受的讲解，根据患者尿道宽窄选用导尿管型号，尊重患者意见选择不同的操作环境，最大限度地体现人文关怀，还应重视应激反应对病情的影响，制定个性化、人性化护理措施，提升护理质量。

Objective To investigate the real - life experiences of peri - operative indwelling catheter in patients with perineal scrub Methods The real - life experiences of 10 patients with indwelling catheter were investigated by phenomenological research methods, a semi - structured interviews was adopted to obtain in - depth interviews. Results Five themes were extracted from interviews ： the lack of related knowledge of perineal scrub indwelling catheter; mental stress, mood swings, irritability; their own pain not be taken seriously and understood ; inferiority and helplessness ; operating conditions affecting the patient experience. Conclusions Operator should pay attention to patients＇feellngs ; width chosen according to the patients＇urethral catheter model; respect the views of patients＇ehoise to adopt different operating environment, maximize reflected humanistic care; should also focus on the impact of stress on the illness to develop personality oriented, humanistic nursing measures to enhance the quality of nursing.
