摘要 目的探讨炎性反应与脑梗死患者颈动脉粥样硬化(CAS)的关系及其在CAS过程中所起的作用。方法选择脑梗死病患者556例,应用彩色多普勒超声检查患者颈总动脉、颈内动脉颅外段及分叉处内-中膜厚度(IMT)。评定标准:IMT≤0.9mm为正常,0.9mm<IMT≤1.5mm为硬化,IMT>1.5mm为斑块形成,管腔狭窄≥30%为狭窄。同时检测血白细胞、纤维蛋白原及其他生化指标,运用逻辑回归来分析相关危险因素对CAS的作用大小。结果年龄、收缩压、纤维蛋白原及白细胞在4组间差异有统计学意义(P<0.05),其中炎性标志物纤维蛋白原及白细胞对CAS的形成及发展影响较大。结论炎性反应与脑梗死患者CAS的形成及发展紧密相关。 Objective To investigate the correlation between inflammation and carotid atherosclerosis (CAS) in patients with cerebral infarction, and the effect intensity of different risk factors on CAS. Methods The carotid intima-media thickness (IMT) was measured by color Doppler ultrasonography in 556 patients with cerebral infarction, according to the results of uhrasonography, the patients were divided into four groups, normal group(IMT≤ 0.9 mm), arterial sclerosis group(0.9 mm 〈 IMT ≤ 1.5 mm), plaque group(IMT 〉 1.5 mm) and stenosis group (stenosis of lumina ≥ 30% ). The leukocyte, plasma fibrinogen and other biochemical markers were detected for all patients. The blood pressure was measured too. Results There were significant differences in the age, fibrinogen, leukocyte and systolic blood pressure among the four groups. The logistic regression showed that all the factors had significant influence on CAS. The inflammation markers, fibrinogen and leukocyte played more important role than the other risk factors. Conclusion In the patients with cerebral infarction,inflammation is significantly correlated with CAS.
