治疗性血管生成联合血管腔内成形术治疗严重下肢缺血性疾病

摘要 目的探讨治疗性血管生成联合血管腔内成形术治疗严重下肢缺血性疾病的方法与疗效。方法对确诊的严重动脉硬化闭塞症患者68例和糖尿病足患者70例,应用肝素3000U,静脉注射,1次/d,连用14d。粒细胞集落刺激因子(G-CSF)300μg肌内注射,2次/d。第3天时采集和分离自体外周血单个核细胞(PBMNCs),配成单个核细胞混悬液,同时行经皮血管腔内成形术并通过导管于缺血部位动脉腔内注入PBMNCs。整个围手术期行功能锻炼。结果全组无死亡,血管腔内成形术即时成功126例,失败12例,即时技术成功率91.30%。术后患肢冷感、间歇性跛行等临床症状不同程度好转。跛行距离由入院时的(24±15)m逐渐提高到12个月时的(642±224)m,表现为持续延长(P<0.01)。溃疡于术后1~3个月逐渐愈合。踝肱指数(ABI)短期明显提升,由入院时的0.60±0.07提高到1个月的0.84±0.11(P<0.01),12个月时略有下降,为0.70±0.07(P<0.05),但仍高于入院时的水平。围手术期无严重并发症和不良反应发生。结论治疗性血管生成联合血管腔内成形术,结合围手术期功能锻炼,对严重下肢缺血性疾病是一种有效的救治方法,具有微创、安全、并发症少、可重复等优点,可作为首选治疗方法。
