胰十二指肠切除术中胰空肠一层吻合术

ONE LAYER PANCREATOJEJUNOSTOMY IN PANCRESTODUODENECTOMY

目的：设计一种在胰十二指肠切除术中能降低病死率和并发症的手术方法；方法：１９９１年１月～９９８年５月胰十二指肠切除术时行胰空肠端端一层吻合１３例，平均年龄５３．２岁。部分病人术前合并较严重的低蛋白血症、贫血和黄疸；结果：全部病人行胰十二指肠切除术，Ｃｈｉｌｄ法重建消化道，空肠胰腺、胆管、胃及Ｂｒａｕｎ吻合均采用一层吻合术。无死亡，无胰瘘等并发症发生；结论：胰腺空肠端端一层吻合是一种操作简便且安全的方法。

Objective:We designed a programe to evaluate the morbidity,mortality and other complications in pancreatoduodenectomy.Methods:From January 1991 to May 1998,We cured 13 patients used one layer pancreatojejunosotomy in pancreatoduodenectomy.The average age of the patients is 53.2 years old.Some of the patients accompanied with hypoproteinemia,anemia and jaundice.Results:All patients was treated by pancreatoduodenectomy,reconstracted digestive tract with Childs method and used one layer pancreatojejunostomy,choledochojejunosotomy,gastroteostomy and jejunojejunostomy with Brauns method.There are not mortality and pancreatic fistula,etc.Conclusion:The method of one layer pancreatojejunostomy is a simple and safe technique of operation.
