自拟黄龙红蛭汤配合西药治疗糖尿病肾脏疾病的临床研究

Clinical Research of Diabetic Renal Disease Treated with the Self-Prepared Huanglonghongzhi Decoction in Combination with Western Medicine

目的研究黄龙红蛭汤配合西药治疗糖尿病肾脏疾病的临床疗效。方法观察100例住院及门诊患者,随机分为治疗组及对照组。对照组采用饮食及运动、口服降糖药物、胰岛素及应用血管紧张素转换酶抑制剂及控制血压、调脂等治疗。治疗组在对照组用药的基础上加用自拟黄龙红蛭汤,每日1剂,疗程为2个月。测定治疗前后24h尿蛋白定量、血清肌酐、血糖、血脂(胆固醇、甘油三酯)及血液流变学检测(全血黏度测定、血浆黏度测定、红细胞变形性测定、红细胞电泳时间测定)。结果治疗组治疗后24h尿蛋白定量(UA)、血清肌酐(Scr)、总胆固醇(TC)、甘油三脂(TG)、空腹血糖(FBG)、餐后血糖(PBG)及全血黏度测定、血浆黏度测定、红细胞变形性测定、红细胞电泳时间测定等指标均下降,与本组治疗前比较,差异有统计学意义(P<0.01);治疗后各生化及血液流变学指标组间比较,差异亦有统计学意义(P<0.05或P<0.01)。结论在西药基础上配合自拟黄龙红蛭汤治疗临床期糖尿病肾脏疾病,疗效明显。

Objective To study the clinical efficacy on diabetic renal disease treated with huanglonghongzhi decoction combined with western medicine.Methods 100 inpatients and outpatients were randomized into treatment group and control group.In control group,the comprehensive therapy was applied,including dietotherapy,physical exercise,the oral administration of anti-glucose drug,insulin,angiotension-converting enzyme inhibitor,as well as blood pressure control and lipid adjustment.In treatment group,on the basis of the treatment as control group,the self-prepared huanglonghongzhi decoction was supplemented,1 dose per day for 2 months as one session.The levels of 24 h UA,Scr,blood glucose,blood lipid（TC,TG）and hemorheologic indexes（whole blood viscosity,plasma viscosity,RBC deformability and RBC electrophoresis time）were detected before and after treatment.Results In treatment group,after treatment,the levels of 24 h UA,Scr,TC,TG,FBG,PBG,and whole blood viscosity,plasma viscosity,RBC deformability and RBC electrophoresis time were all reduced as compared with those before treatment,indicating significant statistical difference in intra-group comparison（P0.01）.After treatment,the significant statistical differences presented in the above biochemical and hemorheologic indexes in comparison between two groups（P0.05,P0.01）.Conclusion The self-prepared huanglonghongzhi decoction supplemented on the basis of western medicine is effective obviously on diabetic renal disease at clinical stage.
