摘要 目的探讨慢性肾功能不全患者心电图表现。方法回顾性分析83例慢性肾功能不全患者心电图。结果 83例患者共做心电图153例次,其中异常心电图120例次(78.4%),以左室肥厚伴劳损多见,共43例次,其次为ST-T改变,共30例次,房性前期收缩8例次,室性期前收缩8例次,窦速7例次,窦缓5例次,传导阻滞2例次,QT延长4例次,低血钾8例次,高血钾5例次。慢性肾功能不全肾功能减退越严重,心电图异常发生率越高(P<0.05))。结论心肌损害为慢性肾功能不全患者常见的并发症和主要死因,其发生机制与尿毒症心肌病变及电解质紊乱等有关。 Objective To analyze the manifestation of electrocardiogram(ECG) in patients with chronic renal insufficiency.Methods The ECG data were retrospectively analyzed in 83 cases of chronic renal insufficiency in our hospital from Dec.2008 and 2009.Results Eighty-three patients underwent a total of 153 events of ECG,in which 120 presented it with abnormal pattem(78.4%).Left ventricular hypertrophy with strain was more common(43 cases) and ST-T changes came the next(30 cases).Premature atrial complex was found in 8 cases and premature ventricular complex in another 8.Seven were sinus tachycardia and 5,sinus bradycardia.Conduction block was seen in 2 QT extension in 4,hypokalemia in 8 and hyperkalemia in 5.Generally,higher incidence of abnormal ECG was seen in severer cases with chronic renal failure(P〈0.05).Conclusion The myocardial damage in patients with chronic renal insufficiency is acommon complication and the leading causes of death whose pathogenesis are aremic cardiomyopathy electrolyte disorders and ofter relevants.
