胰岛素瘤术中腹腔镜超声的定位诊断研究

Localization diagnosis of laparoscopic ultrasonograph in laparoscopic surgery of insulinoma

目的 探讨腹腔镜超声在胰岛素瘤腹腔镜术中定位诊断中的价值.方法 回顾性分析解放军总医院2007年1月至2008年8月经手术证实的32例胰岛素瘤患者的腹腔镜超声结果,对腹腔镜超声的定位诊断准确率进行分析.结果 32例患者中,腹腔镜超声对31例患者的胰岛素瘤成功定位,检出率为96.9%,定位准确性为100%;腹腔镜超声共检出37个胰岛素瘤,其中29例为单发,2例为多发.1例患者反复探查未发现明确胰岛素瘤.结论 腹腔镜超声对胰岛素瘤的检出率高,并可清楚显示肿瘤的毗邻关系,有助于术中选择合适的手术方式.

Objective To discuss the localization diagnosis value of laparoscopic ultrasonography in laparoscopic surgery of insulinoma.Methods Retrospectively reviewed the results of laparoscopic uItrasonography in 32 patients from Jan 2007 to Aug 2008 with insulinoma which confirmed after surgery,and analyzed the detection rate of laparoscopic ultrasonography. Results Insulinoma tumolrs were success located in 31 patients among 32 patients by laparoscopic ultrasonography.The detection rate was 96.9% and the location accurate rate was 100%.Thirty-seven insulinoma tumors were detected in 31 cases,including 29 solitary insulinoma and 2 multiple insulinomas.One patient did not show a definite tumor in the pancresg and extrapancreatic organ during the laparoscopic ultrasonography examination.Conclusion Laparoseopic ultrasonography has a high detection rate in localizing diagnosis of insulinoma and can clear display the relationship between tumor and adjacent tissue.It is useful for surgeon to select the suitable operation modality.
