两种手术方法治疗腋臭疗效观察

The effect of two methods on bromhidrosis

目的探讨梭形切除腋毛区Z成形手术和小切口手术治疗腋臭的效果和并发症。方法 A组（Z成形术）54例,顺腋毛区梭形切除皮肤、部分皮下脂肪,切口做Z成形缝合。B组（小切口术）46例,于腋毛区做两个1cm小切口达皮下,用专用器械在皮下刮去皮瓣内面脂肪组织,破坏大汗腺,直接缝合切口。结果 A组痊愈（气味完全消除）50例,显效（气味明显减轻）4例,回访6个月~5年,无复发,疗效确切。B组痊愈27例,显效17例,无效2例,两组差异有显著性意义（P〈0.05）。A组术后切口二期愈合1例,有明显线形瘢痕2例。B组术后伤口二期或三期愈合7例,局部硬块状瘢痕14例。两组差异有显著性意义（P〈0.05）。结论腋臭梭形切除Z成形术效果确切,并发症发生率低,是治疗腋臭的理想手术方式。

Objective To investigating the effect and complications of Z-plasty and the small incision method on bromhidrosis.Methods 100 patients with bromhidrosis were divided into Group A（54 cases） and Group B（46 cases）.In Group A,the skin and part subcutaneous fat were removed and made a small incision suture of the Z-plasty.In Group B,made two small incision to the subcutaneous area,scraped the adipose tissue inside skin flap,destroyed the large sudoriferous gland and sutured incision directly.Results In Group A,there were 50 cases got rid of the odor completely and the odor of the other 4 cases was significantly alleviated.In Group B,there were 27 cases got rid of the odor completely and the odor of the other 17 cases was significantly alleviated.Conclusion It is effective to treat bromhidrosis with Z-plasty method.
