微型营养评估表和营养筛查表的信度和效度评价 被引量:46

Evaluation of Reliability and Validity of Mini-nutritional Assessment and Chinese Nutrition Screen

摘要 目的评价微型营养评估表(mini-nutritional assessment,MNA)和中文版营养筛查表(Chines enutrition screen,CNS)的信度和效度,为建立适合中国老年患者的营养筛查表提供依据。方法便利抽样法选取112例老年住院患者,每例患者均使用MNA、CNS和营养风险筛选表(nutrition risk screening,RNS)进行评估,检验MNA和CNS的信度和效度。结果 MNA和CNS的内部一致性信度Cronbach’sα系数分别为0.711和0.695;根据Pearson相关分析,在内容效度上,MNA、CNS分别有3个和5个条目与所属因子相关性较差;两量表与效标量表NRS的Pearson相关系数分别为0.661(P=0.000)、0.393(P=0.000);两量表的因子分析均不理想,均出现因子条目载荷量低及与公共因子有偏离的现象;MNA营养良好截断界值的ROC曲线下面积(0.878)高于CNS(0.716)。结论建议在MNA内容基础上进行条目的修改,吸收CNS的部分内容,并根据不同种族、地区老年人的体格、饮食文化特点和生活条件等制定适合中国老年患者的营养筛查表。 Objective To evaluate the reliability and validity of mini-nutritional assessment(MNA) and Chinese nutrition screen(CNS) so as to provide evidence for establishing a suitable nutrition screening scale for the elderly inpatients.Methods MNA,CNS,and nutrition risk screening(NRS) were used to assess 122 elderly inpatients who were selected with convenience sampling.Then the reliability and validity of MNA and CNS were evaluated.Results The Cronbach’s α of internal consistency reliability of MNA and CNS were 0.711,0.695 respectively.According Pearson analysis,there were 3 and 5 items which had bad relevance with their common factors in MNA and CNS,respectively.The Pearson correlation coefficient r of MNA CNS and NRS were 0.661(P=0.000),0.393(P=0.000) respectively.The factor analysis of MNA and CNS were both not satisfying with low load capacity of factor and deviation of public factor.The area of ROC curve of MNA of the interruption value under good nutrition(0.878) was higher than that of CNS (0.716).Conclusion Researchers are suggested to modify some items based on MNA,absorb part items of CNS,then make a favorable nutritional assessment tool according to people’s physical constitution,dietary habit and living condition the elderly patients different races regious.
