摘要
[目的]应用磁共振弥散加权成像(DWI)上表观弥散系数(ADC)分析超急性、急性及亚急性期脑梗死的缺血半暗带范围.[方法]53例脑梗死患者中超急性期者为6例,急性期为18例,亚急性期为29例;给所有患者进行常规MRI,DWI检查,测量病灶平均ADC、相对表观弥散系数(rADC)及病灶旁3,5mm区ADC.[结果]53例脑梗死患者病灶平均rADC为55%~75%;超急性及急性期病灶旁3mm处平均rADC为78%~80%,亚急性期为100%;超急性、急性及亚急性期病灶旁5mm的平均rADC为100%~105%.[结论]脑梗死在超急性期及急性期存在缺血半暗带,其范围为病灶旁3mm区,亚急性期无缺血半暗带.
OBJECTIVE To analyze the infarct ischemic penumbra(IP)range in hyperacute,acute and subacute cerebral infarct by apparent dispersion coefficient(ADC)on magnetic resonance imaging(MRI)and diffuse weighed imaging(DWI).METHODS Of 53 patients with cerebral infarct,6 cases were of hyperacute,18 cases of acute,and 29 cases of subacute.All of patients with cerebral infarct were examined by conventional MRI and DWI,and the average ADC value of lesions,the relative ADC(rADC)and ADC in adjacent to 3,5 mm area of lesion were determined.RESULTS In 53 cases of infarcts lesions,average rADC value was 55%~75%,the average rADC value in adjacent to 3 mm area of hyperacute and acute lesions was 78%~80% and of subacute lesion was 100%,and the average rADC value in adjacent to 5 mm area of hyperacute,acute lesion and subacute lesions were 100%~105%.CONCLUSION IP exist in hyperacute and acute cerebral infarct lesion,the range of IP was adjacent to 3 mm area of infarct lesion,3 mm around the region,and there is no IP in subacute cerebral infarct lesion.
