胃癌手术预防性抗菌药物应用的调查分析

Application of Preventive Antibiotics in Gastric Cancer Surgery

目的探讨胃癌手术预防性抗菌药物的合理使用。方法回顾性地分析604例胃癌手术预防性抗菌药物的使用情况。结果 604例胃癌手术患者全部预防性使用了抗菌药物,76.0%为头孢类抗菌药物,中位用药时间6 d,65.9%联合用药,其中84.4%联合使用硝基咪唑类抗厌氧菌药物,多手术开始后30 min～1 h给药,术中未追加用药,术后87例(14.4%)发生感染性并发症。结论应重视胃癌手术预防性抗菌药物的使用,纠正不合理用药。

OBJECTIVE To evaluate the application of preventive antibiotics in gastric cancer surgery.METHODS The application of preventive antibiotics in 604 cases with gastric cancer surgery was analyzed retrospectively.RESULTS For all of the 604 cases,the preventive antibiotics were administrated.Cephalosporin was used most widely which accounted for 76%.The median duration of administration was 6 days.In 65.9% of the patients,a combined antibiotic regimen was adopted,and among these cases,nitromidazoles were used in 84.4% cases.In most cases,the antibiotics were administrated 30-60 min after the beginning of operation.The postoperative infectious complications occurred in 87 cases（14.4%）.CONCLUSION The rational administration of preventive antibiotics in gastric cancer surgery should be emphasized in order to avoid unreasonable medication.
