摘要
目的探讨非粒细胞缺乏的深部真菌感染患者血浆中(1-3)-β-D-葡聚糖检测的临床价值。方法应用北京金山川公司MB-80微生物动态快速检测系统,及GKT-5 MSet动态真菌检测试剂盒定量检测深部真菌感染组、细菌感染组和健康对照组各30例标本中血(1-3)-β-D-葡聚糖的含量。结果健康对照组血浆中(1-3)-β-D-葡聚糖含量为(0.003±0.002)ng/L;深部真菌感染组为(0.188±0.207)ng/L;细菌感染组为(0.008±0.006)ng/L;经SPSS统计软件进行分析,健康对照组与深部真菌感染组(1-3)-β-D-葡聚糖差异有统计学意义(P<0.01);深部真菌感染组与细菌感染组1,3-β-D-葡聚糖差异有统计学意义(P<0.01);健康对照组与细菌感染组1,3-β-D-葡聚糖差异无统计学意义。结论血浆中(1-3)-β-D-葡聚糖检测可在拟诊早期为临床医师提供机体是否感染真菌的信息,G试验方法检测血浆(1-3)-β-D-葡聚糖可作为真菌感染的一个诊断指标,以0.02 ng/L为诊断阈值其敏感性、特异性较理想。
OBJECTIVE To study the clinical value of(1-3)-β-D-glucan in diagnosis of invasive fungal infection in patients with non-agranulocytosis.METHODS Microbiology Kinetic Rapid Reader MB-80 was used to detect plasma levels of(1-3)-β-D-glucan.RESULTS The mean level of plasma(1-3)-β-D-glucan in healthy people control group was(0.003±0.002) ng/L,in deep fungal infection group was(0.188±0.207)ng/L and in bacterial infection group was(0.008±0.006)ng/L.The plasma(1-3)-β-D-glucan concentration in control group and deep fungal infection group showed statistical difference(P〈0.01).But in healthy group compared with that of bacterial infection group there,was no statisticol difference(P〉0.05).CONCLUSIONS Plasma(1-3)-β-D-glucan can provide reliable information for deep fungal infection in early stage when 0.02ng/L are considered as cut-off,the plasma(1-3)-β-D-glucan provides an ideal sensitivity and specificity.
