钬激光剜除术治疗良性前列腺增生大于100g近期疗效观察

摘要 目的评估前列腺钬激光刻除术(HOLEP)治疗良性前列腺增生(BPH)前列腺大于100g的近期疗效,提高手术安全性和有效性。方法应用钬激光切除术治疗前列腺增生大于100g的BPH患者69例。监测和记录患者围手术期和术后3个月、6个月复查指标,最大尿流率(Qmax)、最大尿流率时逼尿肌压(Pdet/Qmax)、国际前列腺症状评分(IPSS)、生活质量评分(QOLS)等并进行统计学分析。结果手术操作时间平均为(110±15.5)min,手术平均失血50ml,术中输血200ml者1例,切除前列腺组织重平均(81±17)g,无手术死亡。术后69例患者随访3~12个月,IPSS、Qmax、Pdet/Qmax、IPPSS、QOLS都有明显改善。结论对于良性前列腺增生大于100g的患者,钬激光剜除术是一种安全有效的手术方法 。 Objective To evaluate the short-term effect of prostate holmium laser engraving technique(HOLEP) treatment of benign prostatic hyperplasia(BPH) prostate greater than 100 grams,and improve surgical safety and effectiveness.Methods 69 patients were diagnosed with BPH more than 100 grams and were treated with HOLEP.The indices such as Qmax,Pdet/Qmax,IPSS,QOLS were monitored and statistically analyzed perioperatively at the third and sixth month postoperatively.Result The average operative time is(110±15.5) min,the average blood loss is 50 ml,only one patient need blood transfusion 200ml.Resection of prostate tissue weight an average of(81±17)g,69 patients were followed up after 3~12 months,IPSS,QOLS,Qmax,Pdet/Qmax were significantly improved compared with those preoperatively.Conclusions HOLEP is an effective treatment for BPH which is more than 100g.
