成人肱骨远端经关节骨折特制钢板内固定探讨

Investigation of Internal Fixation for Intraarticular Fractures of Distal Humerus in Adult.

目的：探索复杂的成人肱骨远端经关节骨折的治疗。方法：２３例复杂的肱骨远端经关节骨折，按ＡＯ／ＡＳＩＦ分型，Ｂ型８例，Ｃ型１５例。切开复位，分别采用多枚克氏针、松质骨螺丝钉、特制解剖钢板和拉力螺钉等内固定方法，术后良好的功能锻炼。结果：１３个病人平均随访２６个月，按改良Ｒｉｓｅｂｒｏｕｇｈ和Ｒａｄｉｎ功能评定标准，优良率８５％，尚可１５％。结论：作者认为解剖钢板更适合人体解剖特点，有利于骨折愈合和关节重建。

Aim:To study the treatment of complex distal humeral fracture.Method:We collected 23 cases of complex distal humeral fractures from Aug 1992 to Aug.1997.According to AO/ASIF classification,8 cases belonged to type B,15 cases of type C.All those patiants had got open reduction,internal fixation(K wires,cancellous screw,anatomic plate or tension screw)and good postoperative functional exersises.Result:We followed up 13 patients for 26 months and evaluated the extremity function according to Risebrongh and Rafins criterion:good 85%,fair 15%.Conclusion:The anatomic plate is more adaptable to anatomy of bone by which it can promote the healing of fracture and reconstruction of joint,achieve good extremities function.
