双时相门冬胰岛素30针治疗老年2型糖尿病的疗效及安全性

Efficacy and safety of premixed insulin aspart 30 in management of elder cases with type 2 diabetes

目的探讨双时相门冬胰岛素30针治疗老年2型糖尿病患者的疗效及安全性。方法血糖控制欠佳,年龄≥70岁的老年2型糖尿病患者68例,随机分成两组,试验组采用双时相门冬胰岛素30针治疗,对照组采用精蛋白生物合成人胰岛素注射液治疗,共12周,测定治疗前后空腹、餐后2h血糖、糖化血红蛋白的变化以及观察不良反应的发生率。结果试验组治疗12周后空腹血糖、餐后2h血糖和糖化血红蛋白均下降,与治疗前比较,差异均有统计学意义(t分别=5.12、4.35、4.64,P均<0.05);治疗后两组空腹血糖、餐后2h血糖和糖化白红蛋白差值比较,差异均无统计学意义(t分别=1.50、1.04、0.89,P均>0.05)。低血糖发生率试验组6例明显低于对照组18例,差异有统计学意义(χ2=7.64,P均<0.05)。结论在老年2型糖尿病患者中使用双时相门冬胰岛素30针治疗效果好,使用方便,安全性好。

Objective To explore the efficacy and safety of premixed insulin aspart （30% free and 70% protamine-bound, BIAsp 30） in elder patients with type 2 diabetes. Methods Sixty eight elder cases of type 2 diabetes whose blood glucose were not well controlled were randomly divided into two groups, one was treated with BIAsp 30, the other with humilin 70/30 twice-daily for 12 weeks. After adjusted for baseline levels, the values of FPG, 2 h PG, HbA1C were assessed before and after treatment. Side effects were observed. Results The levels of FPG, 2 h PG and HbA1C were descended in BIAsp 30 group after treatment, and the difference was significant compared with that before treatment （t=5.12,4.35,4.64, P〈0.05）; there was no difference between two groups after treatment （t=1.50,1.04,0.89, P〉0.05）. Six cases had hypoglycemia episodes in BIAsp 30 group and 18 cases in the humilin 70/30 group （χ^2=7.64,P〈0.05）. Conclusions Using BIAsp 30 in management of elder cases with type 2 diabetes was effective, convenient and safe.
