假体材料在急性嵌顿性腹股沟疝中的应用

目的:探讨假体材料在急性嵌顿性腹股沟疝急诊手术中应用的可行性。方法:对2004年4月～2008年12月行假体材料腹股沟疝142例患者资料进行回顾性分析。结果:在142例中,有110例择期行假体材料修补术后,其中10例出现阴囊血肿,4例发生腹股沟血肿,5例发生血清肿,3例表皮脓肿,均经局部治疗痊愈。有32例嵌顿疝行急诊网片修补术后,出现阴囊血肿4例,腹股沟血肿及腹壁下积液各1例,5例为绞窄性疝行小肠切除,1例出现无法根治的深部感染致补片失败,未出现其他严重并发症。结论:假体材料在腹股沟嵌顿疝的治疗中有着重要而又有限的作用,应合理选择病例,并尽量避免在小肠切除手术中使用假体材料。

Objective：To investigate the feasibility of the use of prosthetic material for inguinal hemia surgical theatment inemergency.Methods：142patients from April 2004 to December 2008 were treated by the use of prosthetic material for inguinal hemia repair and analyzed retrospectively.Results：142patients were divided into two groups according selective oremergency surgery.Their postoperative complicationsincludedscrotumhaematoma10,inguinalhaematoma4,serumtumidness5,andepidermisadscess3.Thepatients with complications were cured by local therapy.The other 32patients underwent inguinal hemia repair using polypropylene mesh the emergency surgery group,4of which had scrotum haematoma after operation,one had inguinal haematoma and the other had abdominal wall fluidity.They were cured by local treatment.Five patients had small intestine necrosis.One had incurable deep infection cause prosthetic material for inguinal henia repair needed to removed.None had other severity complications.ConclusionIt：Prosthetic material in inguinal hernia the treatment of incarcerated have an important but limited role,should be areasonable choice cases and avoid the use of in the small intestine resection prosthesis materials.
