介绍一种移动乳头的巨纤维腺瘤切除术的切口设计

An Incision Design of Giant Fibroadenoma Ectomy Suit for Nipple Lift

目的介绍一种简易可靠的切口设计,以解决乳腺巨纤维腺瘤引起的乳头移位。方法右乳巨纤维腺瘤患者1例,肿块15 cm×14 cm大小,引起右乳头向外下方移位3 cm,采用自行设计的双柄状的环乳晕切口:切口的两个柄状部分分别位于乳晕的两侧,两个柄状部分的一端和半圆形的环乳晕切口两端相连,该切口通过去表皮操作可以实现乳头的移位。结果本例手术时间1.5 h,出血100 ml,术后12 d出院,移位的乳头位置得到矫正,术后外观效果好,乳头乳晕血运良好。结论本切口设计适用于巨纤维腺瘤等良性病变引起的乳头向外或向下移位的矫正,能达到肿瘤切除和乳头复位的双重目的。

Objective A giant fibroadenoma in the breast can cause nipple transposition.It is a dilemma for a surgeon to return the displaced nipple to the normal position when resects the fibroadenoma.An incision design to solve this puzzle is explored here in the article.Methods The incision introduced was a double stipitiform with either stipitiform linked to an end point of the semi-circle-areola incision.The surgery procedure including design principle was described by a patient underwent this surgery;moreover,the reliable of this incision was demonstrated by reviewing the literature about nipple blood supply.Results Well reveal was obtained by this incision.The incision met the demands of both giant fibroadenoma resection and return the dislocated nipple to the normal position.A breast giant fibroadenoma resection with this incision acquired normal nipple position and a good-looking postoperation view after six month follow-up.Conclusion The indications of this incision include nipple extro-displacement or downward displacement caused by a giant fibroadenoma or other benign lesions in breast.
