白内障超声乳化术并发症及处理

Complications and management in phacoemulsification

目的分析和讨论白内障超声乳化术的手术并发症及其处理。方法对老年性、先天性、并发性和外伤性白内障３００只眼进行了超声乳化并人工晶状体植入术，术后随访患者３～２４个月。结果术中并发症包括切口撕裂、前囊膜瓣残留、虹膜损伤及后囊膜破裂。术后并发症有角膜水肿、复视及黄斑水肿。术后矫正视力０．３～０．５者３４眼，０．６～１．０以上者２６６眼。结论并发症的发生与晶状体核的密度、颜色、术者的操作技术及经验有密切关系。

ObjectiveTo analyse the complications and management of phacoemulsification during and after operation.MethodsPhacoemulsification and I0L were performed in 300 eyes with senile,congenital,complicated and traumatic cataract.The patients were followed up for 3～24 months.ResultsThe complications included tearing of incision,hyphema,damage to the iris,residual tag of anterior capsule,and rupture of posterior capsule during operation,and corneal edema,diplopia,and macular edema after operation.Corrected visual acuity were 0.3～0.5 in 34 eyes and 0.6～1.0 or over 1.0 in 266 eyes after operation.ConclusionOcurrence of complications is related to density and color of cataracts,surgical technique and experience of surgeons.Early identification and correct management of complications after phacoemulsification are important for post operative effect.
