摘要
目的探讨泪囊鼻腔吻合术中遇到的主要难点及其对策。方法通过50例手术的回顾性分析,总结各环节常见难点的原因及采取的相应措施。结果术中难点主要有3个方面。造孔时作入口难10例,筛泡下移4例,粘膜瓣吻合困难而作单瓣吻合7例。出血于3个环节多见,即作皮肤切口,造骨孔、切缝粘膜。每种难点均作妥当处理,并取得良好的效果。本组50例经5个月～2a追踪观察,47例治愈,占94.00%。结论本术主要的难点是造骨孔、止血、吻合粘膜这3个环节。认真分析,妥当处理,是提高本术疗效的关键。
Objective To Discuss the key point and its management in dacryocystorhinostomy. Methods Retrospectively analyze 50 cases of dacryocystorhinostomy surgery, The key point of each operative step was discussed. Results There were 3 key points in the operation. Firstly, it was difficult to make intrance in 10 cases. Ethmoidal bulla down growth in 4 cases. And mucosal flap anastomosis could not be finished in 7 cases. Bleeding had aften occurred when we made skin incision, bone hole, cut and sutured mucous membrane. The key points was managed and the resnlt was satisfactory. 47 cases （94.00%） was cured after 5 months to 2 years of following-up. Conclusion The key points of dacryocystorhinostomy were making bone hole, henostasis and mucosal flap anastomosis. Carefully analyzing and managment are needed to get a better effect.
