利多卡因复合瑞芬太尼硬膜外麻醉在剖宫产中的应用

摘要 目的:探讨利多卡因复合瑞芬太尼硬膜外麻醉在剖宫产中的应用效果。方法:对我院产科2009年1~12月足月妊娠初产剖宫产妇100例,随机分为观察组和对照组,两组均选用硬膜外麻醉,L2~3椎间隙穿刺,观察组给予利多卡因和瑞芬太尼50μg硬膜外腔注入;对照组仅给予利多卡因。记录疼痛消失时间、阻滞有效时间、全程局麻药用量,观察不良反应。结果:与对照组比较,观察组疼痛消失时间缩短,阻滞有效时间延长,全程利多卡因用量减少,差异均有显著性(P<0.05),但两组不良反应发生差异无显著性(P>0.05)。结论:利多卡因联合瑞芬太尼硬膜外麻醉,镇痛效果良好,对母婴无不良影响,值得临床推广。 Objective:To investigate the application of remifentanil combined with epidural lidocaine anesthesia in cesarean section. Methods:From January 2009 to December 2009 100 cases of primiparas with full-term pregnancy and cesarean section were randomly divided into two groups. Both groups used epidural anesthesia,L2-3 intervertebral space puncture.The observation group was given lidocaine and remifentanil 50 μg epidural injection, the control group was given lidocaine. To record disappearance time of pain,the effective block time ,full local anesthetic dosage. Adverse reactions were observed. Results:Compared with the control group ,the pain disappearance.time was shortened ,the effective block time was prolonged,full amount of lidocaine was reduced with significant difference(P〈0.05). But the adverse events of two groups had no significant difference(P〉0.05).Conclusion:Remifentanil combined with lidocaine by epidural administration has the good analgesic effect without adverse effects on mother and infant,which is worthy to be popularized.
