摘要
目的:通过观察补肾壮骨冲剂治疗老年男性骨质疏松症骨转换指标,探索老年性骨质疏松的优势与不足。方法:将老年男性骨质疏松症患者120名,随机分为补肾壮骨冲剂组,仙灵骨葆胶囊组,鲑降钙素针组,固邦片组,进行为期半年的治疗。分别测定服药前及服药半年后患者的骨密度(BMD)、人抗酒石酸酸性磷酸酶5b(TRAP 5b)、骨特异性碱性磷酸酶(BAP)、氨基端和中段骨钙素(N-MID BGP)等骨质疏松症相关指标,比较每组患者服药前后及组间各项指标的变化情况。结果:补肾壮骨冲剂组服药半年后,与治疗前比较其血清TRAP5b减低(P<0.05),BAP及BMP升高(P<0.05),BGP变化不明显(P>0.05)。与仙灵骨葆胶囊组比较:补肾壮骨冲剂组的BAP及TRAP5b改善程度较仙灵骨葆胶囊组明显(P<0.05),BMP升高程度两者比较差异无统计学意义(P>0.05);BGP升高程度不及仙灵骨葆胶囊组明显,服药前后测量值比较差异有统计学意义(P<0.05)。与鲑降钙素针组比较:补肾壮骨冲剂组BMP的升高程度较鲑降钙素针组明显,服药前后比较差异有统计学意义(P<0.05);TRAP,BAP,BGP三项指标两组未见明显差别(P>0.05)。与固帮片组比较:补肾壮骨冲剂组BAP的改善程度不及固邦片明显,服药前后比较差异有统计学意义(P<0.05);BMP,TRAP,BGP三项指标组比较差异无统计学意义(P>0.05)。结论:补肾壮骨冲剂、仙灵骨葆胶囊、固邦片、鲑降钙素针联合钙剂、活性维生素D应用,能促进骨形成,抑制骨吸收,提高BMP,有效防治老年男性骨质疏松症。仙灵骨葆胶囊对成骨细胞的促进作用不如补肾壮骨冲剂明显。补肾壮骨冲剂在提高骨密度方面优于鲑降钙素;对成骨细胞的促进作用不如固邦片明显。
Objective:To observe the curative effect of Bushen Zhuanggu Powder (BZP) on senile osteoporosis, and to study its advantages and shortages.Methods:One hundred and twenty senile osteoporosis patients were randomized into treating group of BZP and control groups of Xianlinggubao, Guijianggaisu, and Gubang, and the therapy lasted half a year. The following indicators were determined before and after using these drugs, such as bone mineral density (BMD), serum tartrate-resistant acid phosphatase-5b (TRACP-5b), bone specificity alkaline phosphatase (BAP), and N-MID osteocalcin (N-MID OT). Results:In BZP group, the level of serum TRACP-5b decreased significantly, while BAP and BMD were remarkably increased (P0.05), but there was no obvious change of N-MID OT (P0.05). The levels of BAP and serum TRACP-5b in BZP group were improved more obviously than in Xianlinggubao group (P0.05), but not the level of N-MID OT. No apparent difference was found in BMD between the two groups (P0.05). Compared to Guijianggaisu group, BMD was obviously elevated in BZP group (P0.05), but there was no significant differences in TRACP-5b, BAP and N-MID OT (P0.05). There was more obvious improvement in BAP in Gubang group than in BZP group (P0.05), but there were no significant differences in the rest indicators between the two groups (P0.05). Conclusion:Bushen Zhuanggu powder, Xianlinggubao, Guijianggaisu, Gubang can promote bone formation, inhibit bone resorption, and elevate bone density while used with calcium and activated vitamin D. BZP can promote bone formation more obviously than Xianlinggubao, and elevate bone density more obviously than Guijianggaisu, but is inferior to Gubang in promoting bone formation.
