螺旋CT及三维成像在诊断胸骨肋软骨骨折中的价值 被引量:22

摘要 目的:探讨多层螺旋CT(MSCT)图像后处理技术在胸骨及肋软骨骨折中的诊断价值。方法:回顾性分析9例胸骨骨折和6例7条肋软骨骨折患者,在工作站进行多平面重建(MPR)、最大密度投影(MIP)、容积再现(VRT)等三维后处理对获得图像进行观察。结果:9例胸骨骨折中,在CT平扫轴位图像上显示6例骨折,而1例胸骨柄骨折、2例胸骨体骨折漏诊,在6例7条肋软骨骨折中1条肋软骨骨折漏诊,三维重建图明确显示9例胸骨骨折和6例7条肋软骨骨折,其中以MPR图显示骨折线最佳,MIP、VRT图显示骨折部位胸骨和肋软骨整体观较佳。结论:MSCT通过MPR、MIP、VRT不同技术相结合多角度观察能够更加完整清晰地显示胸骨骨折和肋软骨骨折部分、形态,提高对细微骨折的显示效果,可为临床提供可靠的诊断依据。 Objective:To probe the value of post-processing techniques of multi-row spiral CT in the diagnosis of sternum and costal cartilage fracture.Methods:Nine sternum fractures and seven costal cartilage fractures in six patients were retrospectively analysed.Post-processing techniques of multiple planar reconstruction(MPR),maximum intensity projection(MIP),and volume rendering technique(VRT) were used.Results:Six of nine fractures were observed on routine axial images,wheras one presternum fracture and two sternal body fractures were missed.One of seven costal cartilage fractures was missed.All of the fractures were clearly seen on three dimension reconstruction images.MPR images prevailed over other images.Anyhow,MIP and VRT were better revealing fracture position.Conclusion:Post-processing techniques such as MPR,MIP and VRT can clearly reveal position and shape of sternum and costal cartilage fracture which may provide detailed information underlying the clinical diagnosis.
