切开复位与闭合复位交锁髓内钉内固定治疗胫腓骨干骨折的疗效比较

Clinical effect comparision of open reduction and closed reduction with intramedullary nail fixation for tibia and fibula fractures

目的比较切开复位与闭合复位交锁髓内钉内固定治疗胫腓骨干骨折的疗效。方法 96例胫腓骨骨折患者随机分为观察组与对照组各48例,观察组采用闭合复位交锁髓内钉内固定治疗,对照组采用切开复位交锁髓内钉内固定术治疗,观察比较2组的疗效、并发症发生率及手术效果。结果观察组总有效率高于对照组,并发症发生率低于对照组,骨痂出现时间、骨折愈合时间、住院时间、手术时间短于对照组,术中、术后出血量明显分别低于对照组,差异均有统计学意义(P<0.01)。结论闭合复位交锁髓内钉内固定治疗胫腓骨干骨折的疗效优于切开复位交锁髓内钉内固定术,可以降低并发症的发生率,缩短手术时间,改善手术效果,值得临床推广和应用。

Objective To compare the clinical effect of open reduction and closed reduction with intramedullary nail fixation for tibia and fibula fractures.Methods 96 patients with tibia and fibular fracture were randomly divided into the observation group and control group,48 cases of each.The observation group was given closed reduction with intramedullary nail,while the control group was given open reduction with intramedullary nail,observed and compared the efficacy of the two groups,complications and results of operations.Results The total effective rate of obserration group was higher,complication rate was lower than the control group,bone callus appeared,fracture healing time,hospital stay,operative time were shorter than control group,intraoperative and postoperative bleeding was significantly lower than the control group,the differences were statistically significant（P0.01）.Conclusion Closed reduction and intramedullary nail fixation of tibia and fibula fracture is better than dry open reduction and intramedullary nail fixation,can reduce the incidence of complications,shorten operative time and improve results of operations,is worthy of clinical and applications.
