31例结外NK/T细胞淋巴瘤(鼻型)患者预后因素分析

Analysis of prognostic factors of 31 cases with extranodal NK/T-cell lymphoma,nasal type

目的：探讨结外NK/T细胞淋巴瘤（鼻型）（ENKTCL）可能的预后因素。方法：回顾性分析31例ENKTCL患者的临床资料,对临床特点、可能的预后相关实验室指标及治疗模式行预后单因素分析。结果：治疗前ECOG体能状态评分0~1的患者占94%,Ann ArborⅠ/Ⅱ期患者占42%,国际预后指数低危患者占39%,低蛋白血症患者占29%。近期疗效完全缓解率48%（13/27例）,中位生存时间28个月。1年、2年和3年的总生存率分别为78%、59%和48%。单因素分析显示治疗前白蛋白水平、治疗模式及近期疗效与ENKTCL患者的预后有关。结论：治疗前白蛋白水平、治疗模式及近期疗效可作为ENKTCL预后的参考因素。

Objective：To investigate the prognostic factors of extranodal NK/T-cell lymphoma,nasal type（ENKTCL）.Method：Records of 31 patients with pathologically confirmed ENKTCL,treated from Aug.1999 to Jul.2008,were reviewed.Detailed clinical and laboratory data and therapeutic modality were included in univariate analysis.Result：Of the 31 patients,42% of the patients were in Ann Arbor Ⅰ/Ⅱ stage.International prognostic index（IPI）in 39% patients were low risk.ECOG performance status（PS）scores in 94% patients were 0-1.And 65% patients presented B symptoms.With a median follow-up duration of 12 months,the median overall survival time was 28 months.The 1,2 and 3 year overall survival（OS）rates were 78%,59%,48%.In univariate analysis,the level of albumin（ALB）,therapeutic modality and short-term response were prognostic factors of ENKTCL.Conclusion：The level of ALB,therapeutic modality and short-term response may be prognostic factors of ENKTCL.
