自动血细胞分析后血涂片复检规则的评估

Analysis and Evaluation of the Experimental Data of Automatic Hematology Analyzer according to International Blood Smear Review Criteria

目的按照血细胞复检规则对雅培CD-3700全自动血细胞分析仪实验数据进行分析、评估。方法用雅培CD-3700全自动血细胞分析仪随机检测患者标本2789份,同时涂片做显微镜检查,按照国际血细胞复检规则进行评估。结果根据血细胞复检规则对检测结果进行统计学分析,真阳性率为9.61%(268/2789);假阳性率为16.57%(462/2789);假阴性率为1.97%(55/2789);真阴性率为71.85%(2004/2789)。由于中国人群血细胞正常参考范围不同于西方人群,通过对数据进行分析,并根据国际血细胞复检规则在贝克曼-库尔特系列血细胞分析仪上应用及改进方案,采用修改后的复检规则和镜检阳性规则评估结果。结论制定合理的适用的血细胞计数和白细胞分类的涂片复检规则非常重要,既可保证检测结果质量,又可提高工作效率。

Objective To analysis and evaluate the experimental data of Abbott Cell Dyn 3700 Automatic Hematology Analyzer according to international blood smear review criteria.Methods 2 789 random-selected blood samples were tested using Abbott Cell Dyn 3700 Automatic Hematology Analyzer,microscopy with smear was made and evaluated according to international blood smear review criteria.Results According to international blood smear review criteria,the true positive rate was 9.61%（268/2 789）,false positive was 16.57%（462/2 789）,false negative was 1.97%（55/2 789）,true negative was 71.85%（2 004/2 789）.Because the reference range of Chinese peripheral blood was different from the Western people and the results were evaluated by using the revised review criteria and the microscopic smear review,according to application and improvement of international blood smear review criteria in Beckman-Coulter Hematology Analyzers.Conclusion It was very important in establishing the suitable and rational criteria for slide review following blood cell count and WBC differential analysis to assure quality of hematology results.It can also enhance work efficiency.
