Academic Thought of LIU Pei-ran

刘沛然(1920-)系全国名老中医,国务院特殊津贴享受者,具有扎实的理论功底和丰富的临床经验。文章从刘老开始的以身试药,尝药470多种,并考据文献打破了细辛限量,用大剂量细辛(120g)治疗脉管炎等多种疑难重奇疾病取得良效,到专著《细辛与临床》(人民卫生出版社出版)、《实用中药歌诀》(人民卫生出版社出版)的出版,以及介绍刘老的用药特点——"四用":重者重用、轻者轻用、血者重用、气者轻用。"四结合":升降结合、动植结合、阴阳结合、动静结合。"一气化":凡用药有重则有轻,如用磁石之重,必用柴胡之轻,亦有自然之气化:如蛇易皮、蟹易鳌。"四个关系":以药带药,以量佐药,量与量矫,药量相调。并举出典型病例从而阐述了刘老的学术思想。

LIU Pei-ran（1920-） is a well-known Chinese Traditional Medicine（CTM） practitioner in China,who enjoys special subsidy from State Council in China. He has both solid basic knowledge in theory and plentiful clinical experience. This essay begins with Senior LIU＇s experience of examining more than 470 kinds of herbs by himself,and he has made a breakthrough in the dosage of Asarum based on numerous investigations on medical documents. He also succeeds in treating difficult and complicated illness like Angitis by administrating large dosage of Asarum（120 grams）. This essay also relates to his monographs of Asarum and clinic （published by People＇s Sanitation Press） and Practical Chinese Prescription in Rhyme（published in People＇s Sanitation Press）. It also elaborates his characteristics in prescribing. Four use：the serious requires more,the lighter requires less,the patient with blood illness needs more while the patient whose Qi（energy） goes wrong needs less. Four combinations：combine increase and decrease,animal and plant,Yin and Yang,dynamic and static. One gasification（a concept in Chinese Traditional Medicine）which means that if there is something heavy in the prescription,something light must be included to complement it. E.g. the Magnetite and the Bupleurum. This phenomenon in nature is snakes molting easily and crabs often renewing its chela. Four relations：using one ingredient to enhance the efficiency of another dose;administrating the dosage according to the seriousness of a patient,neither too much nor too less;different ingredients should comply with each other considering their properties. Besides, many typical clinical cases are listed in this essay to elaborate Senior LIU＇s academic thought.
