64层螺旋CT灌注成像在脑星形细胞瘤分级诊断中的价值 被引量:3

摘要 目的探讨CT灌注成像在星形细胞瘤分级诊断中的价值。方法脑星形细胞瘤患者21例,入院后常规进行CT灌注成像,术后病理常规行组织切片及免疫组化检查。其中低级别星形细胞瘤7例,高级别星形细胞瘤14例。结果各级别星形细胞瘤的局部血流量(rBF)、局部血流容积(rBV)、平均通过时间(MTT)和表面通透性(PS)均高于对侧正常脑白质(P<0.05),高级别星形细胞瘤的rBF、rBV和PS均明显高于低级别星形细胞瘤(P<0.05)。结论 CT灌注成像有助于脑星形细胞肿瘤的术前分级,可为临床治疗提供参考。 Objective To evaluate the value of CT perfusion in the diagnosis and grading of astrocytoma before operation.Methods Twenty one patients with astrocytoma received CT examinations including plain scan,enhanced dynamic scan and enhanced scan.The perfusion 3.0 software was used for the dynamic data analysis;The regional blood flow(rBF),regional blood volume(rBV),mean transit time(MTT) and permeability surface(PS) was calculated and the correlated mapping was acquired.The HE and immunohistochemistry staining of the specimen were performed for pathology.Results There were 7 cases of low-grade(WHO,gradeⅠ~Ⅱ) astrocytoma and 14 cases of high-grade(WHO,gradeⅢ~Ⅳ) astrocytoma.The value of rBF,rBV,MTT and PS in astrocytoma were significant higher than those in normal white matters(P〈0.05;the values of rBF,rBV and PS in high grade astrocytoma were higher than those in low grade astrocytoma with great significance(P〈0.05).Conclusion CT perfusion imaging is useful in grading of astrocytoma before operation and can help offer useful information forclinical management.
