热疗联合FOLFIRI化疗治疗晚期结直肠癌的临床观察

Clinical Observation of Thermotherapy Combined with FOLFIRI Chemotherapy for Advanced Colorectal Cancer

目的:观察热疗联合FOLFIRI全身化疗治疗晚期结直肠癌的疗效和不良反应。方法:将2005年5月～2009年8月在我院住院治疗的53例晚期结直肠癌患者,随机分为A、B两组,A组(治疗组,26例)接受热疗联合FOLFIRI全身化疗,肿瘤局部区域热疗,温度43℃,每次60分钟,每周2次;B组(对照组,27例)接受常规FOLFIRI全身化疗。化疗方案为伊立替康180mg/m^2,静脉滴入90 min,d_1;亚叶酸钙200mg/m^2,静脉滴入2h,d_(1,2);5-氟尿嘧啶400mg/m^2,静脉推注,d_(1,2),600mg/m^2,持续静滴22h,d_(1,2);14天为1个周期;治疗4个周期后,评价疗效及不良反应。结果:与常规全身化疗相比,热疗联合全身化疗组(A组)有效率、疾病无进展生存时间(PFS)及总生存期(OS)有明显提高,差异有统计学意义(P<O.05),不良反应未增加。结论:对晚期结直肠癌患者,热疗联合FOLFIRI化疗疗效较好,不良反应能耐受。

Objective： To observe the efficacy and adverse reactions of thermotherapy combined with FOLFIRI systemic chemotherapy for advanced colorectal cancer. Methods： 53 patients with advanced colorectal cancer were randomly divided into A, B groups. A group （ treatment group, 26 cases） received FOLFIRI systemic chemotherapy combined with thermotherapy; B group （ control group, 27 cases） received only FOLFIRI systemic chemotherapy. FOLFIRI regiment was as follows： irinotecan 180mg/m^2 intravenous infusion 90 min, dl; leucovorin 200mg/m^2 intravenous infusion 2h d1,2; 5-fluorouracil 400mg/m^2 intravenous injection d1,2, 600mg/m^2 , continuous infusion 22 h d1,2 ; every 14 days for one cycle. A group received thermotherapy at local tumor, temperature to 43℃, for 60 minutes, twice a week. The efficacy and adverse reactions were evaluated after four cycles. Results： Thermotherapy combined with systemic chemotherapy was superior to chemotherapy in effective rate, progression-free survival （PFS） rate and overall survival （OS） rate. Conclusion： Thermotherapy combined with FOLFIRI systemic chemotherapy is safe and effective for patients with advanced colorectal cancer.
