置管引流联合尿激酶、糜蛋白酶治疗老年结核性胸腔积液临床分析

The treatment of elderly tuberulous pleural effusions with the micro-wound drainage through a thin tube and urokinase and chymotrypsin

目的探讨置管引流联合尿激酶、糜蛋白酶治疗老年结核性胸腔积液,防治胸膜粘连、肥厚的疗效。方法在抗结核治疗的同时,将49例老年结核性胸腔积液患者随机分成两组,实验组23例,予以微创置管引流,同时胸腔内注射尿激酶、糜蛋白酶治疗;对照组26例,予以反复胸腔穿刺抽液治疗。结果与时照组比较,治疗组效果明显,胸膜粘连、肥厚发生率明显降低,肺功能改善明显。结论置管引流联合尿激酶、糜蛋白酶治疗老年结核性胸腔积液的疗效肯定、安全、实用,能够有效防治胸膜粘连、肥厚发生、改善肺功能,值得在临床上推广应用。

Obejective To inquire into the curative effect of the treatment of elderly tuberulous pleural effusions with the micro-wound drainage through a thin tube and urokinase and chymotrypsin,and against protection pleural thicking and adhension.Methods Under the antituberculotic treatment.49 patients of tuberulous pleunisy were divided randomly into the treatment group（23 cases）and the control group（26 cases）.The micro-wound drainage in the treatment group were performed with a thin tube with the injection of urokinase and chymotrypsin,but the control group just drainage.Results Compared with control group,the curative effect of the treatment group is apparent,the rate of pleural thicking and adhension decreased significantly,and the function of the lung improved significantly too.Conclusion The treatment of elderly tuberulous pleural effusions with the micro-wound drainage through a thin tube and urokinase and chymotrypsin is a safe and practical method.It has a satisfactory.It is able to protect against pleural thicking and adhension,and it can improve the function of the lung.It is worthy of being recommended for a wide use in clinical practise.
