青海西宁地区490例大肠息肉的临床及病理分析

Pathological and Clinical Features of Colorectal Polyps in Xining Area:An analysis of 490 cases

目的：分析大肠息肉患者好发年龄、部位、病理类型间的关系。方法：对电子肠镜检查中检出的不同年龄段大肠息肉患者的临床表现、部位、病理类型进行分析总结。结果：2 451例结肠镜检查病例共检出大肠息肉490例,总检出率19.9%,男性检出率21.2%（289例）,女性检出率18.5%（201例）。其中直肠175例（35.7%）,乙状结肠109例（22.2%）,左半结肠99例（20.2%）,右半结肠66例（13.5%）,多部位41例（8.4%）。腺瘤性息肉239例（48.8%）,其中管状腺瘤156例（31.8%）,绒毛状腺瘤51例（10.4%）,混合性腺瘤32例（6.5%）;非腺瘤性息肉251例（51.2%）,其中增生性息肉125例（25.5%）,炎性息肉121例（24.7%）,幼年性息肉5例（1%）。结论：大肠息肉高发于直肠和乙状结肠,随着年龄的增加而增加,60岁以上患者的发病率明显升高,病理类型以腺瘤性息肉最为多见,其中管状腺瘤在（40~59）岁年龄组较多,而绒毛状腺瘤及混合性腺瘤在60岁以上组较多。息肉部位与性别、年龄之间无相关性。

Objective：To investigate whether the pathological features of colorectal polyps occur in proper relation to ages of onset and location in patients with colorectal polyps.Methods：A total of 2 451 patients（men 1 364,women 1 087;aged 14 to 69 years） between Sep.2000 and April 2008 in our hospital,who underwent colonoscopy,were collected and screened in this report.490 of 2451 cases had colorectal polyps;and the total detective rate is 19.9%,men in 21.2%（289 cases）,and women in 18.5 %（201cases）.The ages and location of onset,and pathological classify were retrospectively analyzed in 490 cases.The T test and One-way ANOVA is done using software SPSS10.0.Results：That the colorectal polyps was common in over 60 years age group.Of 490 cases of colorectal polyps,175 cases（35.7%） was in rectum,109 cases（22.2%） in sigmoid,99 cases（20.2%） in left colon,66 cases（13.5） in right colon and 41cases（8.4%） in multiple sites.The rate of adenomatous polyp and non-adenomatous polyp was 48.8% and 51.2% respectively.Conclusions：The incidence of colorectal polyps had a positive relation with ages,and over 60 ages people are more easy to suffer from it,the common symptom of colorectal polyps is hematochezia.It occurs easier in rectum and left colon than in the other site.The common pathological type is adenomatous polyp,and there is no connection among pathogenic sites and ages,gender.
