糖尿病视网膜病变与尿微量蛋白的关系

The relationship between microproteinuria and diabetic retinopathy

目的探讨尿微量蛋白与糖尿病视网膜病变（ＤＲ）的关系。方法检测１０６例糖尿病病人尿微量蛋白的４个指标：ＡＬＢ／Ｃｒ、ＴＲＦ／Ｃｒ、ＲＢＰ／Ｃｒ和ＮＡＧ／Ｃｒ，同时观察这些病人的视网膜情况。结果伴有糖尿病视网膜病变的病人的ＡＬＢ／Ｃｒ、ＴＲＦ／Ｃｒ、ＲＢＰ／Ｃｒ和ＮＡＧ／Ｃｒ较不伴有糖尿病视网膜病变的病人明显增高。经Ｌｏｇｉｓｔｉｃ回归分析，ＴＲＦ／Ｃｒ作为独立的因素仍然与糖尿病视网膜病变相关。结论ＴＲＦ／Ｃｒ的异常可能预示着早期的糖尿病视网膜病变。

ObjectiveTo investigate the relationship between microproteinuria and diabetic retinopathy(DR).MethodsThe urinary microalbumin excretion index(ALB/Cr),urinary transferrin excretion index(TRF/Cr),urinary retino binding protein excretion index(RBP/Cr) and urinary N acetyl beta,D glucosaminidase excretion index(NAG/Cr)were compared between 106 patients with and without DR.ResultsALB/Cr,TRF/Cr,RBP/Cr and NAG/Cr were found obviously high in diabetic retinopathy patient group. After Logistic regression analysis, TRF/Cr is still associated with diabetic retinopathy as an independent factor.ConclusionThe abnormality of TRF/Cr may be an early indicator of DR. The larger scale investigation and multi factor analysis are needed to confirm this result.
