血清LDH水平对非霍奇金氏淋巴瘤的预后作用初探

目的：探讨血清LDH水平对非霍奇金氏淋巴瘤（NHL）预后估计的意义以及与AnnArbor分期、组织学分型的关系。方法：采用乳酸酶为底物的酶促反应法测定55例NHL患者外周血清中LDH水平。结果：55例NHL患者中LDH水平升高组与LDH水平正常组2年总生存率分别为57％、88％，4年总生存率分别为27％、71％；完全缓解（CR）率分别为66％、96％，部分缓解（PR）率分别为21％、4％，P值均＜0．05。Ⅰ＋Ⅱ期与Ⅲ十Ⅳ期NHL患者中LDH值差异不显著（P＞0．05）。低度恶性与中度恶性NHL的LDH值差异不明显（P＞0．05），而高度恶性与中度恶性、低度恶性NHL的LDH值差异显著（P＜0．05）。结论：LDH水平可作为一个独立的预后因素来预测NHL，LDH水平高者预后差。

Purpose: To investigate the significance of dictate Dehydrogenase(LDH) level in predicting NHL and its correlation with Ann Arbor stage and histological type. Method: Serum level of LDH in 55 NHL patients was measured by enzymatic synthesis method that used lactate as substrate. RusSults: In 55 NHL patients, the 2 － year and 4 － year overall surval rates were 57% and 27% in high LDH level group, 88% and 71% in normal LDH level group. n'he complete and partial 'remission rates were 66% and ZI% in high LDH bevel group, 96% and 4% in normal LDH level group. There were significant difference between the two groups(P < 0. 05). Comparing with Ⅰ+Ⅱ stage NHL patients, the LDH level Of Ⅲ +Ⅳ stage patients had nosignificant difference (P > 0. 05 ). The LDH level had no significant deference (P > 0. 05) between low aide and intermediate grade group, but there were significance difference between high grade and intermediate grade group or high grade and low grade group(P < 0. 05). Conclusion: The LDH level is an independent prognostic factor for NHL. The LDH level is higher, the prognosis is more unfavourable.
