摘要
目的分析海拔因素对脓毒症大鼠血清中细胞因子表达和肺损伤的影响,探讨细胞因子网络与脓毒症及某些高原疾病的相关性。方法将果洛州玛多组(4268m)、西宁组(2276m)和郑州组(112m)各54只雄性SD大鼠随机分为正常对照组(n=6)、假手术组(n=24)和模型组(n=24)。采用盲肠结扎穿孔术(CLP)制备脓毒症模型,于造模后2、6、12、24h处死大鼠,假手术组在相同时间点处死大鼠,采用双抗体夹心酶联免疫吸附法(ELISA)测定血清TNF-α、IL-6、IL-10水平;留取各时间点完整左肺组织做病理切片,右肺组织测定湿干(W/D)比重;记录模型组各时间点脓毒症大鼠死亡例数,统计其死亡率。结果模型组各时间点TNF-α、IL-6、IL-10水平均高于假手术组(P<0.01);各组间相比较,随着海拔升高,炎症细胞因子各时间点表达水平均明显升高(P<0.01);肺组织损伤情况呈递进性加重;模型组于造模6h以后肺湿干比重差异显著(P<0.01);模型组脓毒症大鼠死亡率明显增加(P<0.05)。结论模型组各时间点细胞因子表达水平均高于假手术组;海拔越高,炎症细胞因子表达水平越高,肺组织损伤越严重,脓毒症症状较低海拔者越重,脓毒症大鼠死亡率越高。
Objective To investigate cytokines and lung injury at different altitudes in rats with sepsis and research the relevance of cytokine network with sepsis and some kinds of plateau disease.Methods The rats were randomly divided into groups of Maduo group(4268m),Xining group(2276m)and Zhengzhou group(112m);each group's 54 male SD rats were randomly divided into control group(n=6),sham operation group(n=24)and CLP group(n=24).The rat model of sepsis induced by cecal ligation and puncture(CLP)in each group were sacrificed at various time points(2h,6h,l2h and24 h)after CLP,Sham group rats were killed at the same time points.The level of the TNF-α,IL-6 and IL-10 were examined with ELISA;specimens from the left lung at each time point were performed pathology slice in order to determine the right lung organization wet/dry(W/D)ratio to assess lung injuries.Record the number of each model group at each time point the number of died cases with sepsis to calculate the mortality rate.Results The model group's TNF-α,IL-6,IL-10 were higher than sham-operated group(P〈0.01)at different time points;compared with higher altitudes among the three groups,the inflammatory cytokines expression at different time points were significantly higher(P〈0.01);slices showed that at higher altitudes,the greater the scope of impairment,the much severe the disease,lung injuries presenting progressive increase;model group at 6 h after modeling the lung wet/dry(W/D)ratio was significantly different(P〈0.01);a significantly increased mortality in model group septic rats(P〈0.05).Conclusion At different time points,model group's cytokine expression were higher than sham-operated group;the higher altitude,the higher the expression levels of inflammatory cytokines;lung injury more serious;sepsis symptoms worsened;a significantly increased mortality in model group septic rats.
