产科出血院前救护管理模式临床研究

Clinical research on the mode of pre-hospital first-aid management of obstetric hemorrhage

目的：探讨高效快捷的产科出血院前救护管理模式。方法：选择2005年1月～2007年10月新的院前急救管理模式接入的产科出血患者47例为实验组，选择2003年1月～2005年9月旧的院前急救管理模式接入的产科出血患者51例为对照组。比较在不同的院前救护模式中其院前急救反应时间上的差异，观察患者在住院天数、并发症发生方面的情况。结果：实验组平均出车速度及5km内、5km外平均院前急救反应时间均快于对照组（P〈0．05，P〈0．01），且实验组患者的住院日缩短（P〈0．05），无产科出血并发症。结论：加强对院前急救时效性的管理、缩短急救半径、尽早针对产科出血的原因采取救护措施，可有效节约院前急救反应时间，保障母婴安全，减少产科出血并发症的发生。

Objective ： To evaluate the efficient mode of pre - hospital first - aid management of obstetric hemorrhage. Methods ： 47 patients with obsteiric hemorrhage admitted to the hospital from January 2005 to October 2007 with a new mode of pre - hospital first - aid management were taken as experimental group and another 51 patients admitted to the haospital from January 2003 to September 2005 with the old pre - hospital first - aid management were taken as control group. The difference in reaction time was compared when the different models of pre - hospital first - aid management were adopted, observing the days of hospi- talization and the occurrence of complications. Results ： The average turnout time of ambulance and reaction time of pre - hospital first - aid within and outside 5 km was faster in the experimental group than that in the control group （ P 〈 0. 05, P 〈 0. 01） ; the duration of hospitalization was shorter in the experimental group than that in the control group （ P 〈 0. 05 ） and no complications of obstetric hemorrhage occurred. Conclusion ： Strengthening time management of pre - hospital first - aid, reducing the radius of first aid and taking proper measures as soon as possible according to the reasons for obstetric hemorrhage can effectively save the reaction time of pre - hospital first aid to ensure the safety of mother and baby and reduce the complications of obstetric hemorrhage.
