Effects of desflurane on hemodynamics in patients during general anesthesia

摘要 目的 观察地氟醚对全麻病人血液动力学的影响。方法 择期手术病人 3 0例 ,ASA 级者 15例 ( 组 ) ,高血压者 15例 ( 组 )。芬太尼、依托咪酯、维库溴铵诱导后 ,插入多普勒气管导管 ,观察不同 MAC地氟醚对两组 MAP、SVR、HR、CVP、CI的影响。结果 两组 MAP、SVR,随地氟醚MAC的升高呈剂量依赖性下降 ,在 2 .0 MAC时 组比 组下降显著。 HR、CVP在 1.0 MAC时无变化 ;在 1.5 MAC及 2 .0 MAC时 ,呈剂量依赖性增加 ,且 2 .0 MAC时 组比 组增加明显。两组的 CI在任何 MAC时均无显著变化。结论 地氟醚对心血管系统影响较小 。 Objective To investigate the effects of desflurane on hemodynamics in general anesthetized patients Method Thirty patients,scheduled for elective surgery requiring general anesthesia,were studied Fifteen patients, ASA grade Ⅰ,were allocated in group I, while the other fifteen patients who suffered from hypertension in group Ⅱ Anesthesia was induced with fentanyl,etomidate and vecuronium intravenously,then endotracheal Doppler intubation were performed,followed by inhalation of desflurane MAP,HR,CI,CVP and SVR were measured and recorded in both groups at 0 5 MAC,1 0 MAC,1 5 MAC and 2 0 MAC of desflurane respectively Results MAP and SVR declined in dose dependent way in both groups, and decreased more significantly in group Ⅱ than in group Ⅰ No significant changes occurred in HR and CVP between both groups at 1 0 MAC,while HR and CVP increased in dose dependent way in both groups and increased more significantly in group Ⅱ than in group I at 2 0 MAC There were no significant changes in CI in both groups at any MAC of desflurane Conclusion Desflurane may maintain the stabilization of hemodynamics during general anesthesia,but high concentration desflurane should be avoided in patients with hypertension
