延长速度对骨延长区修复组织的影响

Effacts of Lengthening Rates on Repaired Tissues of the Lengthened Zone.

延长速度是肢体延长术基本问题之一，它对骨延长区修复组织有很大的影响。３６只兔行左胫骨上干骺端截骨延长，通过组织学检查，观察１ｍｍ／ｄ、２ｍｍ／ｄ和３ｍｍ／ｄ（Ａ、Ｂ、Ｃ３组）３种不同延长速度对延长区骨修复组织的影响。结果发现，Ａ组延长区修复组织细胞生长活跃，骨细胞演化过程未受明显阻碍。Ｂ、Ｃ２组延长区修复组织内创伤反应大，牵伸过程中延长区内出现大片出血灶，同期骨修复组织成熟度不如Ａ组。作者认为，延长速度是影响延长区修复组织骨演化过程关键因素之一，延长速度以每日１ｍｍ为宜，其低牵张力有利于骨细胞的演化及成熟。

The basic problem in limb lengthening is the rate of elongation which has effects on bone repaired tissues of the lengthened zone. In this paper the elongation of proximal metaphyseal osteotomy of tibial was performed on 36 rabbits. Through the observation of histology, the effects of 3 lengthening rates (1,2 and 3mm/ day for the animals of Groups A B and C respectively) on repaired tissues of the lengthened zone were investigated. The results showed that the repaired tissues cell is growing actively, the process of bone cell erolution has not hindered in Group A. The large hemorrhagic focus and wound reaction occurred to the lengthened region in traction process in Group B and C. The matured degree of the repaired tissues in B and C are not so good as A. It is believed that lengthening rate is a key factor to bone healing of lengthened zone. The lengthening rate of 1mm/day is suitable and low traction is favorable for the bone cell to evolution and maturity.
