树突状细胞疫苗的制备及对胃肠道恶性肿瘤患者免疫功能的影响

摘要 目的:研究疫苗对胃肠道.恶性肿瘤患者免疫功能的影响。方法:从人外周静脉血中分离单核细胞,人重组粒细胞/巨噬细胞集落刺激因子(rhGM-CSF)和人重组白细胞介素4(rhIL-4)、IL-2、和IFN-γ诱导扩增,CEA相关多肽致敏,促树突状细胞成熟以提高其抗原提呈能力。双相倒置显微镜下观察树突状细胞的形态结构,流式细胞仪检测树突状细胞细胞表型。成熟的树突状细胞经皮下和静脉两种回输途径回输给胃肠道恶性肿瘤患者。观察回输疫苗后一个月、三个月患者细胞免疫及体液免疫的变化。结果:双相倒置显微镜观察,培养第7天的悬浮细胞大部分具有典型树突状细胞形态特征,细胞表面粗糙,有大量皱折和不规则突起。培养第7--15d增殖最明显,培养15d,树突状细胞纯度达85%以上。流式细胞仪检测树突状细胞中度表达CDla,高表达CD86,CD40,HLA-DR。病人的细胞免疫及体液免疫指标CD4、CD8、CD3、CD4/CD8、IgG、IgM明显提高,IgA略有提高,CEA有所下降。结论:树突状细胞疫苗给胃肠道晚期恶性肿瘤患者回输后一个月、三个月,患者细胞免疫CD4,CD8和体液免疫IgG,IgM具有显著提高。 Objective: To investigate effect of the vaccine on patient's immunocompetence against gastroenteral cancer. Methods: The mononuclear cells isolated from human peripheral venous blood were cultured in the supplement of rhGM-CSF, rhIL-4, IL-2 and WN-γ , then sensitized with CEA-related polypeptide. Matured dendric cells with improved ability of antigen presentation were observed under inverted microscope of the morphology and structure, and were presented by flow cytometry. Mature dendric cells were given to the patients of gastric colorectal subcutaneously or venously. The cellular immunity and humoral immunity were detected at one month and three month after dendric cell was used. Results: Most of the suspension cells had typical dendric cell's morphological characteristics as rough and wrinkle surface under inverted microscope. Dendric cell proliferation was dramaticly significant in 7-15days. Purity of dendric cell reach to more than eighty-five percent by 15th day. Dendric cell presented morderately mark such as CD1 and presented highly mark such as CD86 ,CD40,HLA-DR when analysed by flow cytometry. The patients' index of cellular immunity and humoral immunity rised apparently in such as CD4, CD8, CD3, CD4/CD8, and a little as lgG, IgM, IgA and went down with CEA. Conclusion: Dendric cell vaccine improved cellular immunity CD4,CD8,and humoral immunity IgG, IgM, IgA of patients with gastroenteric tumor.
