慢性乙型肝炎患者外周血单个核细胞共刺激分子和FOXP3 mRNA表达的研究

摘要 目的探讨外周血单个核细胞共刺激分子和(CD_4)^+(CD_(25))^+调节性T细胞FOXP3 mRNA表达在慢性乙型肝炎患者免疫耐受中的作用。方法以β-actin为内参,用RT-PCR和Real-Time PCR方法对61例慢性HBV感染者(其中免疫清除期患者37例、免疫耐受期患者24例)和20例健康对照组外周血单个核细胞共刺激分子B7-2、PD-L1及FOXP3 mRNA的表达水平进行检测,并分析PD-L1/B7-2比值与FOXP3的相关性。结果外周血单个核细胞PD-L1/B7-2比值在慢性乙型肝炎患者免疫清除期和免疫耐受期均明显高于健康对照组;FOXP3 mRNA表达在免疫清除期明显高于免疫耐受期和健康对照组(P<0.01);免疫清除期PD-L1/B7-2比值与FOXP3表达呈明显正相关。结论慢性乙型肝炎患者正负共刺激分子和FOXP3 mRNA的表达有相关性;共刺激分子和(CD_4)^+(CD_(25))^+调节性T细胞可能共同参与调节机体对HBV感染的免疫耐受。 [Objective] To explore the roles of eo-stimulatory molecules B7-2, PD-L1 and FOXP3 mRNA expression of peripheral blood mononuclear cells in immune tolerance of chronic hepatitis B virus infection. [ Methods] RT-PCR and Real-Time PCR methods were applied to determine the expression of B7-2, PD-L1 and FOXP3 mRNA in peripheral blood mononuelear cells in 61 HBV-infected patients and 20 healthy controls. The relation of PD-L1/B7-2 ratio and FOXP3 was analyzed. [Results] compared with healthy individuals, PD-L1/B7-2 ratio was significantly up-regulated in both immune-active and immune-tolerant adults. FOXP3 mRNA expression in the immune-active patients was higher than in the immune-tolerant adults and healthy controls. PD-L1/B7-2 ratio was positively correlated with FOXP3 mRNA expression in the immune-active patients. [ Conclusion ] In chronic HBV infection, the expression of co-stimulatory molecules was associated with expression of FOXP3 in the immune-active patients; co-stimulatory molecules and CD4^+CD25^+ Treg may play an important role in synergistically regulating the immune reaction in chronic HBV infection.
