纤维支气管镜在困难胃管置入危重患者中的应用

Application of fiberoptic bronchoscope in stomach tube indwelling in serious patients

目的评价纤维支气管镜在困难胃管置入危重患者中的应用价值。方法 187例常规胃管置入困难危重患者采用纤维支气管镜引导置入和纤维支气管镜明视置入胃管,观察胃管置入成功率和并发症。结果 187例患者经纤维支气管镜引导置入胃管135例,均1次成功置入,无明显鼻咽部黏膜损伤并发症;行纤维支气管镜明视置入胃管52例,1次成功置入49例,3例改为纤维支气管镜引导成功置入。结论对预测或常规胃管置入困难患者应用纤维支气管镜引导或明视置入,能明显提高胃管置入成功率,减少并发症。尤以纤维支气管镜引导法为佳。

【Objective】To evaluate the value of fiberbronchoscope application to help stomach intubation in serious patients.【Methods】187 critical patients with the difficulty of stomach intubation routinely were included. The tube intubation was conducted or under the fiberoptic bronchoscope. The success rate and complications were observed. 【Results】135 cases were installed successfully only once guided by the fiberoptic bronchoscope and no complication happened. 52 cases were installed under the optic help of the fiberbronchoscope,49 cases were carried out only once,3 cases failed and changed to indwell by the guiding of fiberoptic bronchoscope. 【Conclusions】The success rate of stomach tube indwelling in the critical patients with tube installing difficulty can improve obviously by the guiding of the fiberoptic bronchoscope,and the complication can reduce markedly. The method of guiding by fiberoptic bronchoscope is our especially recommended.
