摘要
目的探讨彩色多普勒超声扫查无名动脉分叉处的临床意义。方法对85例经CT或MRI检查后,临床确诊的缺血性脑中风患者进行彩色多普勒颈动脉及无名动脉扫查,并与对照组134例同年龄组非缺血性脑中风患者的颈动脉及无名动脉扫查结果相比较。结果缺血性脑中风组的颈总动脉分叉处及无名动脉的内中膜厚度(IMT)及斑块检出率明显高于对照组,且无名动脉的内中膜厚度及斑块检出阳性率明显高于颈总动脉分叉处,此外两组均发现了无名动脉扩张疾病,且缺血性脑中风组的无名动脉扩张检出率高于非缺血性脑中风组,P<0.05。结论颈动脉及无名动脉的动脉硬化及粥样斑块与缺血性脑中风密切相关,且无名动脉病变的相关性更为紧密,无名动脉分叉处的动脉硬化及斑块发生率高于颈总动脉分叉处,其检查更有助于发现动脉硬化和粥样斑块的存在,从而减少漏诊,并有助于其他相关疾病的诊断,其临床意义不可忽视。
Objective To explore the clinical significance of color Doppler sonography study in innominate artery bifurcation.Methods A total of 85 cases with cerebral infarction as confirmed by CT or MRI were studied with color Doppler sonography of carotid artery and innominate artery.The results were compared with that of 134 cases of the same age group without cerebral infarction as the control group.Results Intima media thickness(IMT) and positive detection rate of carotid and innominate artery plaques in the cerebral infarction group were much higher than that in the control group.Intima media thickness and positive detection rate of innominate artery plaques were higher than that of common carotid artery bifurcation.Moreover,innominate artery dilatation was found in two groups and the positive detection rate of innominate artery dilatation in the cerebral infarction group was higher than the control group(P0.05).Conclusion There was a close relationship between arterial sclerosis and plaques of the carotid and innominate artery and the cerebral infarction.In addition,the relationship of innominate artery was closer than carotid.The incidence rate of arterial sclerosis and plaques of innominate artery were higher than that of common carotid artery bifurcation.The color Doppler sonograms of innominate artery helped to find out the existence of arterial sclerosis and plaques and decrease rate of missed diagnosis.It also helped to diagnosis other diseases concerned.Color Doppler sonograms of innominate artery was an important method with unnegligible clinical value.
