胃癌根治术后免疫型肠内营养支持的应用 被引量:3

摘要 目的:为了探讨免疫营养制剂在胃癌术后营养支持上的作用,通过使用含特殊营养物质的免疫增强型肠内营养制剂与术后早期标准肠内营养的比较,研究术后早期免疫肠内营养对胃癌患者营养状态、免疫功能、应激状况的影响,评价免疫肠内营养应用的优势。方法:将48例胃癌病人随机分为两组,每组24例。第l组术后早期给予常规肠内营养制剂(EN)(对照组),第2组术后早期接受免疫增强型肠内营养制剂(IEN)(实验组),均在肠内营养输注系统通过空肠造口管持续泵入肠内营养剂,连续7天。于术前、术后第1天、术后第9天分别检测两组病人的营养指标和免疫指标。结果:两组病人术后第9天营养指标、免疫指标均较术后第1天有升高(P〈0.05);实验组营养指标及免疫指标较对照组显著升高(P〈0.05);48例病人均能较好地耐受肠内营养支持。结论:免疫增强型肠内营养制剂较常规肠内营养制剂更有利于增强胃癌术后病人的营养及免疫功能。 Objective: In order to explore the immune Nutrition in gastric surgery on the role of nutritional support,by the comparison of use of special nutrient-containing immune-enhancing enteral nutrition and early standards enteral nutrition after the operation,we study the influence after the early postoperative enteral nutrition on gastric cancer patients' nutritional status,immune function and stress condition,to evaluate the advantages of the immune enteral nutrition applications.Methods: Forty-eight patients with gastric cancer were randomized into 2 groups,everyone has twenty-four patients in it.The group 1 of early postoperative enteral nutrition with routine preparations(EN)(control group).The group 2 received early postoperative immune-enhancing enteral nutrition(IEN)(test group),were fed the immune-enhancing general nutrition after radical operation through jejunostomy for seven consecutive days.In patients before and after 1 day,9 days after the two groups were measured indicators of the patient's nutritional and immune indicators.Result: The postoperative nutrition state and immune response in the two groups was better than preoperative(P0.05).And the postoperative nutrition state and immune response in test group is better than those in contro1group,the differences were significant.The result of 48 patients who accepted enteral nutrition support is good.Conclusion: Immune-enhancing enteral nutrition compared with conventional enteral nutrition in gastric cancer is more conducive to enhancing the patient's nutrition and immune function.
