DHS治疗股骨粗隆间骨折失败的原因分析 被引量:4

摘要 目的总结应用髋动力加压螺钉(DHS)治疗股骨粗隆骨折的失败病例,探讨失败原因。方法采用回顾性分析的方法,回顾2008年10月~2009年10月105例股骨粗隆间骨折DHS内固定的病例,骨折按Evans标准分类,骨质疏松按Singh标准分度,根据术后X线片评定手术的成败,即内翻角>100°,股骨头切割,螺钉穿出超过20mm或内固定物断裂认定为手术失败。结果本组105例手术患者中,10例失败,发生率为9.5%。其中髋螺钉切割致穿出股骨头2例(1.9%),螺钉拔出、钢板松动4例(3.8%),普通螺钉断裂3例(2.9%),钢板折断1例(0.9%)。按照Evans分类法,稳定骨折中60例有4例失败,不稳定骨折中45例中有6例失败。按Singh骨质疏松分类中,治疗失败10例均属Ⅰ、Ⅱ和Ⅲ级患者。结论股骨粗隆间骨折的Evans分类法对该病治疗有一定的指导意义,术前同时进行骨质疏松Singh分级有利于对应用DHS治疗该病进行预后判断有一定参考价值。对于不稳定的粗隆间骨折,并伴有骨质疏松的病例,容易发生内固定失败,选择DHS时应慎重。
