摘要 目的乳腺癌发病率近年来呈增加的趋势,早期发现、早期诊断、早期治疗对降低死亡率,提高生存率具有重要意义,文中探讨三维超声容积成像在乳腺肿块诊断中的应用。方法在二维超声检查的基础上,对57例乳腺肿块用容积探头进行三维成像,并与手术后病理检查结果对比。结果三维容积成像可提高对乳腺肿块边缘阳性声像特征的检出,可在冠状断面上清晰地观察到恶性病灶对周边组织浸润的特殊征像"太阳征",二维和三维超声对此征像的检出差异有统计学意义(P<0.05)。三维彩色能量成像(three-dimensional color Doppler energy,3D-CDE)可清晰地显示肿块血供的空间分布,有助于鉴别乳腺肿块的性质。结论三维容积成像克服了二维超声仅能对超声束平行的平面成像的限制,可清晰、真实地显示肿块内部的细微结构,获得的病变信息更接近实际解剖结构,是诊断乳腺肿块的有价值的方法 。 Objective The incidence of breast cancer is on the rise in recent years.Early diagnosis and early treatment count significantly to the improvement of survival of the patients.This study aimed to explore the value of three-dimensional (3D) volume ultrasound in the diagnosis of breast masses.Methods A total of 57 breast masses were examined by 3D volume ultrasound based on the information obtained by 2D ultrasonography.And results were compared with those of postoperative pathology.Results The 3D imaging clearly demonstrated the stereoscopic characteristics of the breast masses,and achieved a significantly higher rate of detection of the discordant boundary echoes than 2D (P〈 0.05).It clearly displayed the special "sun-like symptom" of malignant infiltration into the peripheral tissues on the coronal plane.Additionally,3D color Doppler energy imaging well manifested the features of blood flow distribution,which helped to determine the nature of the breast masses.Conclusion Three-dimensional volume ultrasound can clearly display the microstructure of tumors,and is valuable for the differentiation of breast masses.
