手术治疗腰椎间盘突出症临床分析106例

Clinical Analysis of 106 Patients of Lumbar Disc Hernia With Surgical Treatment

目的探讨腰椎间盘突出症手术治疗效果。方法采用后入路"开窗"或半椎板或全椎板切除法治疗腰椎间盘突出症106例,并对手术效果进行评价。结果本组106例中共随访88例,随访率83.0%。采用Macnab评估标准,优73例(83.0%),良11例(12.5%),优良率95.5%。结论准确的诊断和定位是手术成功的保证,手术应在彻底减压并摘除髓核的基础上,尽量减少对腰椎结构的破坏,以保持腰椎的稳定性。

Objective To discuss the surgical curative effect of lumbar disc hernia. Methods 106 patients of lumbar disc hernia were treated by opening through the back entry or removing half or whole lamina of vertebral arch. The surgical curative effect has been evaluated. Result 88 patients in this group have been followed up. The rate of following is 83.0%. 73 patients are excellent （83.0%） and 11 are better （12.5%） by adopting Macnab assess criteria. The rate of excellent and better is 95.5%. Conclusion The def inite diagnosis and position are guarantee to success for operation. The operation Should reduce the damage on lumbar vertebrae structure to the least on the base of reducing hypertension thoroughly and removing nucleus pulposus to maintain the stability of lumbar vertebrae.
