肾囊肿腹腔镜手术与开放手术、介入治疗对比分析及临床研究

The therapeutic effect of laparoscopic renal cyst unroofing and renal cyst paracentesis with open renal cyst unroofing.

目的比较后腹腔镜肾囊肿去顶术与开放性肾囊肿去顶术、介入穿刺注入硬化剂治疗的疗效，并探讨后腹腔镜肾囊肿去顶术处理要点。方法回顾性分析26例后腹腔镜肾囊肿去顶术和21例开放性肾囊肿去顶术及11例介入穿刺注入硬化剂治疗患者的临床资料，比较各组手术入路、手术时间、术中损伤、术中失血量、术后止痛剂使用、术后下床活动时间、术后住院天数和复发率等指标。结果各组比较，后腹腔镜手术组手术时间、术中失血量及术后住院时间均显著少于开放性手术组（P〈0．01）。后腹腔镜手术组复发率显著少于介入治疗组（P〈0．01）。结论后腹腔镜肾囊肿去顶术具有创伤小，患者康复快、疼痛轻、复发率低的优点，治疗单纯性肾囊肿的疗效明显优于开放性手术及介入穿刺注入硬化剂治疗。

Objective To compare the therapeutic effect of laparoscopic renal cyst unroofing and renal cyst paracentesis with open renal cyst unroofing. And to approach the high light of laparoscopic renal cyst unroofing treeatment. Methods The clinical date of 26 cases treated by laparoscopic renal cyst unroofing and 21 cases receiving open renal cyst unroofing and 11 cases receiving renal cyst paracentesis were retrospectively analyzed. Compare the index such as operative route, operation time, damage, blood loss, use of paregoric, out - of - bed activity time, post - operation hospital time and recurrence rate. Results Operation time of laparoscopic and openrenal cyst unroofing had nosignificant different. The amount of bleeding during operation, the indwelling cathetertime, the drainage tube time, the fasting time, the hospital stay were all better than the open surgical （P 〈 0.01 ）. Recurrence rate of the laparoscopic renal cyst unroofing is more less than the paracentesis （P 〈 0. 01 ）. Conclusion The laparoscopic renal cyst unroofing has the advantages of less trauma and pain and rapid recovery, and it is better than the open renal cystunroofing. Especially the laparoscopic operation have combination superiority than the open operation and the paracentesis.
