非哺乳期乳腺炎微创治疗的初步探讨

目的探讨非哺乳期乳腺炎微创治疗的临床应用价值。方法 38例经穿刺抽吸或穿刺活检诊断为非哺乳期乳腺炎的女性患者,在超声引导下使用麦默通行乳腺炎病灶微创清除术及超声引导下病灶清除加置管冲洗治疗。结果 38例患者中32例采用麦默通微创手术,共旋切36处病灶,超声均能清晰显示病灶微创旋切的及脓液抽吸全过程;6例体表有溃疡窦道者于彩超引导下行病灶及窦道搔刮;术后复查有2例2个病灶未被完整切除,完整切除率94.4%,1例患者创面血肿形成。38例患者病理结果阳性率100%。结论超声引导微创治疗非哺乳期乳腺炎具有定位切除准确、病理诊断率高、创伤小、疗效好、操作简单、安全等优点,值得推广。

Objective To evaluate the value of application of ultrasound-guided treatment for non-lactation mastitis. Methods The micro-invasive operations for 38 patients were performed by ultrasound-guided Mammotome system and ultrasound-guide therapeutic catheter drainage respectively which confirmed by biopsy beforeoperations. and the therapeutic results were evaluated. Results All the operations were accomplished by ultrasoundguided.6 patients with fistula formation were treated with surgical excision. and the other 32 cases were performed by ultrasound-guided Mammotome system. There were 2 cases found with postive margins for inflammatory granulomatous tissue and 1 patient had postoperative haematoma. Conclusion Ultrasound-guided micro-invasive operations can be used in treatment of non-lactation mastitis. It is an efective technique with microinvasion. it can be equally applied for the excision of leisions.
