萎缩性胆囊炎的腹腔镜切除技术探讨

Technology Investigation of Laparoscopic Cholecystectomy of Atrophic Cholecystitis

目的:探讨萎缩性胆囊炎的腹腔镜切除术的方法及技术要点。方法:回顾性分析97例萎缩性胆囊炎的临床资料。结果:患者全部治愈,84例在腹腔镜下完成,13例中转开腹。术后2例发生胆漏,经引流治愈。无术后出血,无切口疝、腹腔感染、肠梗阻等近期并发症。结论:萎缩性胆囊炎的腹腔镜切除术手术难度大,术中保持解剖清晰、手术技巧灵活,结合具体情况采取合适的治疗方法,术后放置腹腔引流管以预防出血和胆汁漏,多数萎缩性胆囊炎患者经腹腔镜胆囊切除术(LC)治疗后获得满意疗效。

Objective：To investigate the method and technology of laparoscopic cholecystectomy of atrophic cholecystitis.Methods：There was the retrospective analysis of clinical data in 97 cases with atrophic cholecystitis.Results：All patient were cured,84 cases had fulfiled under laparoscopic cholecystectomy,13 cases turnned to disclose abdomen cholecystectomy.There were 2 cases taking place bile leakage in postoperation,which were cured by abdominal draining.There was no complication such as bleeding、incisional hernia、abdominal infection and bowel obstruction after operation.Conclusion：Laparoscopic cholecystectomy of atrophic cholecystitis is difficulty,it is important to keep anatomy clear,grasp operation skill and to combine with specific condition to adopt suitable operation.Should place abdominal cavity drainage tube in postoperation to prevent bleeding and bile leakage,most of laparoscopic cholecystectomy of atrophic cholecystitis may obtain satisfactory treatment effect.
