摘要
目的:探讨老年单纯收缩期高血压(ISH)患者左心室肥厚(LVH)形成的相关因素。方法:收集未用过降压药物并排除重大疾病的老年高血压患者160例,分为单纯收缩期高血压组(ISH组)及双期高血压组(SDH组)两组,并与64例同期健康老年人作为健康对照组比较。应用彩色超声多普勒技术测定左室肥厚(LVH)水平、放射免疫方法测定三组血管活性物质(ANP、Aid及AngⅡ)参数。结果:两个高血压组舒张功能受损指标与对照组有显著性差异(P〈0.01),以ISH组明显;。钠素(ANP),醛固酮(Aldo),血管紧张素(AngⅡ)水平在两个高血压组明显升高,而降钙素基因相关蛋白(CGRP)水平则较健康对照组下降(P〈0.01)。结论:ISH患者易出现舒张功能受损,其血管活性物质的异常参与LVH形成。
Objective To investgate the relation between Isolated systolic hypertension (ISH) in patients and left ventricular hypertrophy (LVH) . Methods collect 160 cases in elderly hypertensive patients , including the ones who had not taken antihypertensive drugs and excluding the ones who had significant diseases. They were divided into two groups;Isolated systolic hypertension (ISH)group, and Systolieand diastolic hypertension (SDH) group; They were compared with healthy elders in the same age as the control group. Level of LVH was detected by Color Doppler ultrasound techniques, three parameters of vasoactive substances (ANP, Ald, and Ang Ⅱ ) were measured by radio--immunity methods. There were significant differences in diastolic dysfunction index between two hypertension groups and Control groups( P 〈0.01), especially the ISH group. Results ANP. Aldo, Ang Ⅱ levels were significantly elevated in both hypertension groups,CGRP level decreased, compared with the control group ( P 〈0.01). Conclusion ISH patients were prone to diastolic dysfunction, the abnormal vasoactive substances were involved in the formation of LVH .
